This letter is an example of a Termination Letter and includes all of the elements of a perfect Termination Letter for you to use. This sample Termination Letter can be used as a template for your needs.
Regretfully, we are terminating your employment at ForeverYoung, effective 7/16/10.
As covered in last month’s meeting, we have yet to see active participation on your part to adhere to company regulations regarding attendance policy. As noted in three previous write-ups from manager Tracy Adams, you have failed to show for the following shifts:
Friday 5/14/10, scheduled shift 4-10pm
Friday 5/28/10, scheduled shift 10-5pm
Friday 6/25/10, scheduled shift 5-10pm
We have gone over the importance of covering your scheduled shifts with you, both individually and at team meetings. It is imperative that our management team can rely on each employee’s commitment to follow through with their shifts once the schedule is posted.
Your final paycheck will be mailed out tomorrow to the following address we have on file for you:
4532 Seacrest Ln.
Portland, ME 04210
We ask for the return of your team member badge or a deduction of $5 will be made from your final paycheck earnings.
Best in your future endeavors,
